Kanamycin monosulfate.
Description
Each capsule contains kanamycin monosulfate with suitable diluent in gelatin capsules.
It is reported that kanamycin monosulfate has the molecular formula of C18H36N4O11·H2SO4·H2O and is composed of desoxystreptamine, 6-deoxy-6-amino-d-glucose and another aminohexose kanosamine.
Action
Microbiology: Kanamycin is a water-soluble, basic antibiotic discovered from the culture broth of a new species of Actinomyces, Streptomyces kanamyceticus.
Kanamycin is active against both in vitro and in vivo gram-positive and gram-negative bacteria as well as acid-fast bacteria (Mycobacterium tuberculosis). It strongly inhibits the growth of these bacteria and exerts a powerful protective effect against infections due to them. Kanamycin has been proven, under the experiments with mice, rats, dogs, cats, goldfishes and monkeys, to be a substance of less acute and chronic toxicity.
Kanamycin shows no cross-resistance with other antibiotics commonly used. Staphylococcus, Escherichia coli and Shigella which have developed resistance to other antibiotics are sensitive to kanamycin. Mycobacterium tuberculosis resistant to one or more of streptomycin, PAS and INH also responds to kanamycin.
It often exerts a favourable effect on infections due to Pseudomonas aeruginosa and Proteus vulgaris, so far considered to be intractable.
Toxicology: There appeared no toxic signs in monkeys that received continuous administration of 200 mg (potency)/kg daily for 50 days.
Indications/Uses
Kanamycin Capsule Meiji is taken orally and is hardly absorbed from the alimentary canal; therefore, it is not for systemic use. Suppression of intestinal bacteria before bowel operations; adjunct in the treatment of hepatic coma.
Dosage/Direction for Use
For Bowel Sterilisation: Adults: 1 g/hr for 4 hrs, and then 1 g/6 hrs for 36-72 hrs. Children and Infants: 150-250 mg/kg body weight/day in divided doses at 6-hourly intervals.
As Adjunct in the Treatment of Hepatic Coma: Adults: 8-12 g/day in divided doses.
The dose should be changed depending on the symptoms. In hunger, it should be taken with the suitable amount of lukewarm water or non-irritating drinks.
For children, it is recommended to wrap the contents of the capsule in an oblate, or to dissolve it in a small amount of lukewarm water, milk or syrup. The effects will be more definite when the administration is continued for 1-5 days after the main symptoms eg, fever, diarrhea and abdominal pain, disappear. The dose should be increased if effects are not observed within 48 hrs. The sign of effectiveness is usually observed in 1-3 days.
Patients with serious symptoms should have the IM injection of Kanamycin Sulphate Meiji at the same time.

For intestinal tract infections eg, bacillary dysentery, acute diarrhea, etc, usually 50-100 mg (potency)/kg body weight/day administered orally in 4 equally divided doses.
Contraindications
There is no evidence that kanamycin is contraindicated; nevertheless, it is ineffective against the infections due to insusceptible pathogens. Hypersensitivity to kanamycin.
Special Precautions
Kanamycin capsules should be used only for oral administration.
Kanamycin should be administered by or under the supervision of a physician.
Particular care is essential for patients with renal and hearing impairment, intestinal ulcer, poor oral and parenteral nutrition. Caution is also necessary when administered in elderly and pregnant and lactating women.

Adverse Reactions
Kanamycin produces no significant side reactions in the treatment of acute infections.
Rarely, loose feces, local pain, headache, numbness and urinary casts are encountered, which are all mild, seldom necessitating cessation of medication. These reactions disappear by suspending the administration.
Rarely, ototoxicity, nephrotoxicity, deafness, vitamins K and B deficiency may occur. Infrequently, gastrointestinal disorders occur.
Drug Interactions
Combined use with anaesthetics or muscle relaxants may aggravate respiratory depression.
